Comprehensive pediatric dental care — from routine checkups to specialist orofacial myotherapy, tongue tie treatment, and jaw development therapy.
Orofacial Myotherapy
Retraining facial and oral muscles for healthy breathing, jaw growth & facial development
Orofacial myotherapy is a specialised therapy programme that retrains the muscles of the face, mouth, and throat to function correctly. Many children develop compensatory muscle patterns due to mouth breathing, tongue tie, habits like thumb sucking, or structural issues. Over time, these abnormal patterns affect jaw development, facial growth, speech, sleep quality, and dental alignment.
As a certified orofacial myotherapist, Dr. Rachayta Gala assesses each child individually and creates a customised programme of exercises and strategies to correct these patterns. Treatment may include exercises for the lips, tongue, and facial muscles, nasal breathing retraining, and targeted myofunctional therapy for swallowing.
The goal is lifelong healthy oral function that supports a beautiful, healthy face and smile — from the inside out.
Signs Your Child May Need Myotherapy
- Breathing through the mouth (day or night)
- Snoring or restless sleep
- Tongue thrust when swallowing
- Narrow or underdeveloped jaw
- Crowded or crooked teeth
- Dark circles under the eyes
- Long, narrow facial appearance
- Difficulty chewing or swallowing
- Teeth grinding (bruxism)
Tongue Tie (Ankyloglossia) Assessment & Treatment
Comprehensive evaluation for infants, toddlers, and older children
Tongue tie occurs when the lingual frenulum — the band of tissue connecting the tongue to the floor of the mouth — is too tight or short, restricting normal tongue movement. In infants, tongue tie can make breastfeeding painful and difficult. In older children, an undiagnosed tongue tie can cause speech difficulties, dental spacing problems, poor jaw development, and can even affect sleep and breathing.
At Milky Whites, we carry out a thorough functional assessment — not just whether tongue tie is present, but how much it is affecting your child’s function and development. We provide both assessment and treatment, and work closely with lactation consultants, speech therapists, and ENT specialists where needed.
Signs to Watch For
- Difficulty latching or painful breastfeeding (infants)
- Speech difficulties or unclear articulation
- Unable to stick tongue out past lips
- Difficulty licking an ice cream or moving tongue sideways
- Dental spacing or crowding issues
- Mouth breathing or snoring
- Restricted jaw development
Mouth Breathing Correction
Helping children transition to healthy nasal breathing
Children who breathe through their mouths — rather than through their nose — are at higher risk of narrow jaw development, dental crowding, poor sleep quality, behavioural issues, and changes to their facial structure over time. Many parents don’t realise mouth breathing is happening because it often occurs during sleep.
Dr. Gala assesses the causes and effects of mouth breathing in each child — including any structural contributions — and creates an individualised treatment plan. This may include myofunctional therapy, nasal breathing exercises, referral to ENT for underlying nasal blockage, and oral appliance therapy where indicated.
Signs Your Child May Be a Mouth Breather
- Lips frequently open or apart at rest
- Snoring or noisy breathing during sleep
- Waking frequently at night
- Dark circles under the eyes despite enough sleep
- Narrow upper jaw or high arched palate
- Crowded front teeth
- Dry lips, chapped mouth corners
- Difficulty concentrating at school
Preventive Dentistry
Stopping dental disease before it starts
Prevention is at the heart of everything we do at Milky Whites. We believe that dental disease is largely preventable — and that a child who grows up with a healthy mouth has a significant advantage in health and confidence throughout their life.
Our preventive services include professional fluoride treatments, fissure sealants (a protective coating applied to the biting surfaces of back teeth), dietary counselling, personalised oral hygiene instruction, and early cavity detection using the latest technology. We work with each family to create a preventive plan that suits their child’s specific needs and risk profile.
Our Preventive Treatments Include
- Fluoride varnish and fluoride supplements
- Fissure sealants (protective coating for back teeth)
- Personalised brushing and flossing instruction
- Dietary advice and sugar counselling
- Early cavity detection with diagnostic tools
- Custom mouthguards for sports
- Night guards for teeth grinders
Early Orthodontics & Jaw Development
Guiding jaw and tooth development from age 7
The best time to address jaw and bite problems is while the face is still growing — typically between the ages of 6 and 12. Early orthodontic evaluation allows us to identify narrow jaws, underbites, overbites, crossbites, crowded teeth, and other developmental concerns before they become complex and expensive to treat.
At Milky Whites, we specialise in interceptive orthodontics — using functional appliances, myofunctional therapy, and careful guidance to help the jaws develop correctly. This approach often significantly reduces or eliminates the need for complex braces treatment later. We follow the Indian and American orthodontic association recommendation for a first assessment at age 7.
Signs Your Child May Need Early Orthodontic Assessment
- Early or late loss of baby teeth
- Crowded or overlapping teeth
- Top teeth biting behind lower teeth (underbite)
- Large gap between upper and lower teeth
- Asymmetric jaw or facial growth
- Thumb sucking after age 5
- Difficulty biting or chewing food
Special Needs Dentistry
Compassionate, expert dental care for every child
Every child deserves excellent dental care — including children with special healthcare needs. Milky Whites has extensive experience and advanced training in treating children with autism spectrum disorder, cerebral palsy, Down syndrome, intellectual disabilities, and other conditions.
Children with special needs often have a greater need for dental treatment — due to increased susceptibility to decay, medications that affect oral health, difficulty maintaining oral hygiene, and higher rates of oral trauma. We offer both careful in-clinic management using specialist behaviour techniques, and treatment under general anaesthesia where required.
We Have Experience With
- Autism Spectrum Disorder (ASD)
- Cerebral Palsy
- Down Syndrome
- Intellectual and developmental disabilities
- ADHD and sensory processing difficulties
- Children requiring treatment under General Anaesthesia
Habit Correction
Thumb sucking, tongue thrusting, and other oral habits
Certain oral habits — if they continue beyond the appropriate age — can alter the development of the jaw, teeth, and facial structure. Thumb sucking, finger sucking, tongue thrusting, and prolonged pacifier or bottle use are the most common. The earlier these habits are addressed, the less impact they have on dental and facial development.
At Milky Whites, we take a child-friendly, positive approach to habit correction. We use a combination of counselling, motivational strategies, myofunctional therapy, and — when needed — custom habit-breaking appliances to help children stop harmful habits comfortably and permanently.
💬 Ask Us About Habit CorrectionSpace Maintainers
Preserving space when a milk tooth is lost early
When a milk tooth is lost earlier than its natural shedding time — due to decay, trauma, or necessary extraction — the surrounding teeth can drift into the empty space. This causes crowding and misalignment of the permanent teeth waiting to erupt underneath.
Space maintainers are small, custom-made appliances that hold the space open until the permanent tooth is ready to come through. They are one of the simplest and most effective preventive tools in pediatric dentistry — often preventing the need for orthodontic treatment later.

Treating cavities and infections in milk and permanent teeth
When cavities do occur, we use the most advanced, minimally invasive techniques to restore the tooth with as little discomfort as possible. We use tooth-coloured composite fillings that blend in naturally with the tooth. For deeper infections, we offer pulp therapy (pulpectomy and pulpotomy) to save the milk tooth until its natural shedding time — avoiding early loss and the need for a space maintainer.
Our behaviour management techniques ensure that even anxious children can have their treatment completed comfortably, without fear, and often without even needing an injection.
